If you are a new patient who has not yet completed the registration form, please submit the required documents below. Please have ready a photo/scan of your PA Driver’s License and a photo/scan of your PA Medical Card.

A consultation is NOT required for new patients to register and make a purchase. If you have already had a consultation at another dispensary or you wish to waive your initial consultation, please fill out the new patient registration form below. Follow up consultations can be scheduled at a later date.

If you are a returning patient who has already completed registration, you are good to go! Feel free to order now.

To visit our dispensary and purchase marijuana medication, the following forms are required for first-time patients only. If you have already visited this location, there is no need to submit additional paperwork. Check out our Pennsylvania locations at our Lancaster dispensary, Thorndale dispensary and the Plymouth Meeting dispensary . As well as our Keystone Canna Remedies locations at our Bethlehem dispensary, Allentown dispensary, and Stroudsburg dispensary.
At this time, consultations are not required to purchase medical marijuana. Consultations are available for patients FREE of charge and can be scheduled online.
